From public radio stations to fashion companies, customers from all over the world are using quizzes to generate qualified leads. When new leads are generated, the creator of the quiz has their responses alongside all of the questions as well as their final outcome.

And now, you can get more than just email addresses.

You can add as many fields as you like to the lead capture form; including, but not limited to first name, last name, phone number, job title, etc. You can customize the button label as well.

So now you can turn this:

To set this up, just go to the “Configure” step in the quiz editor, and click on “Offer”. Then, make sure “Form” is selected from the options in the drop down menu located in the upper left hand corner of the quiz, and add any additional fields that you want to be a part of the lead capture form. IMPORTANT: If you have the Mailchimp integration enabled, be sure to fill in the "map to:" section (click the ellipsis) so Mailchimp can fill the correct field with the information captured in the lead form.
